The Result Card and T.C. from the school last attended. If the TC has not been taken, a bonafide certificate issued by the school showing the class and date of birth should be attached.
Birth Certificate issued by the Competent Authority (the original should be deposited in the school at the time of admission and it will not be returned till the child leaves the school).
Proof of residence of Father / Mother to be submitted.
Disability Certificate from the Civil Surgeon.
