• Copy of Adhar card of the child (compulsory)

• Registration Form duly completed and signed by Parents / Guardians

• Bus requisition form duly completed (if bus service required)

• State whether Schedule Caste / Schedule Tribe / OBC. Yes/No

If yes attach copy of a Caste Certificate
