• Keep your details of Debit Card/Credit Card/Net banking ready for online payment of Rs. 25/- as the registration amount.
• In the case of the single parent, the phone no. of the parent is to be repeated in both the columns.
• Fill up the form completely before taking a printout. In case of incomplete form/documents, the application will be deemed as rejected.
• Print out of the filled registration form, duly signed by the parent, must be submitted at the fee counter, along with self-attested photocopies of the supporting documents.
• Non-submission of the filled registration form along with self-attested supporting documents will make you non-eligible for the draw for the final list.
• The last date for submission is 27.12.2019 up to 02:00 PM.

• Date of Birth Certificate of the Applicant from Municipal Corporation.
• Proof of Residence (Aadhar card / Ration Card / Electricity / MTNL bill / VoterID/Passport)
• Proof of Sibling, if applicable – Latest fee receipt.
• Proof of Alumni, if applicable – Matriculation Certificate.
