Please submit attested copies of the following documents along with the form: (i) Copy of the Birth Certificate (Self Attested) (ii) Medical Fitness Certificate by a doctor (in original) (iii) Child's & Parent's Aadhar Card(self-attested) (iv) Proof of residence (Aadhar card / Rent Agreement / License/ Voters ID/ Passport/ Electricity / Gas / MTNL Bill) issued in the name of one of the parents. (v) Proof of transfer, if any,(In case the parents have been transferred in the last academic session) (vi) SC/ST/OBC Certificate( If applicable)3. In case of admission to Class II or above, please also enclose: (i) Photocopies of the last year Progress Report Card (ii)Transfer certificate from the previous school (original)4. Sibling Proof: Latest fee receipt/copy of ID Card of the sibling studying in the school.5. The school buses ply on the specific routes only.6.
